New Patients

New patients can register in the following ways:

  • You may call into the surgery bringing with you your NHS medical card from your previous doctor
  • If you cannot find your medical card or if you do not have one, you may still call in at the surgery to register but will need to complete the appropriate forms.  This method of registering is less preferable as it may take longer to trace your medical records.
  • You may register online via the “Registering Online” tab above. You will need to do this for each member of your family.  When we receive your details we will complete the paperwork and write to you asking you to attend the surgery to sign the forms and bring in your proof of ID.


A combination of the following can be accepted as identification (it is preferable that one item of photo ID is seen, along with one document containing the patient’s address):

  • birth certificate
  • marriage certificate
  • medical card
  • driving licence
  • passport
  • local authority rent card
  • paid utility bills
  • bank/building society cards/statements
  • National Insurance number card
  • payslip
  • letter from Benefits Agency/benefit book/signing on card
  • papers from the home office
  • P45.

This list is not exhaustive. The following documents are easily obtained but will not be accepted as proof of identity if presented in isolation:

  • library card
  • video rental card
  • health club card
  • private rent book.

If we are unable to accept your registration, for example if you live outside our practice area, we will inform you accordingly.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.
