New Patients
Before filling out a registration form, check you are in our catchment area.
New patients can register in the following way
- Find a GP – NHS (www.nhs.uk). You will need to do this for each member of your family. When we receive your registration we will check everything is correct then we will contact you to bring in your identification and book a new patient health check.
Identification
A combination of the following can be accepted as identification (it is preferable that one item of photo ID is seen, along with one document containing the patient’s address):
- birth certificate
- marriage certificate
- medical card
- driving licence
- passport
- local authority rent card
- paid utility bills
- bank/building society cards/statements
- National Insurance number card
- payslip
- letter from Benefits Agency/benefit book/signing on card
- papers from the home office
- P45.
This list is not exhaustive. The following documents are easily obtained but will not be accepted as proof of identity if presented in isolation:
- library card
- video rental card
- health club card
- private rent book.
If we are unable to accept your registration, for example if you live outside our practice area, we will inform you accordingly.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.
Temporary Residents
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
